We help navigate the next level of care for your patients, specific to their needs, with the idea of minimizing hospital readmissions.  We do this by assisting in the transition to a community where they can receive 24/7 care specific to their needs, manage their diseases/chronic illnesses with medication management, & increase their socialization where they will thrive in their new environment. 

 

Transitional Care:

Whether your patient is in need of Independent Living, Assisted Living, Memory Care, or Nursing Home care, we can help guide them through this complicated process free of charge in 3 easy to follow steps:

  1. Meet the patients at their home or by phone to do a  complete assessment to determine appropriate level of care needs, amenities, desired location & affordability.

  2. Set up tour, accompany patient/family on tour of facility, & assist with making a decision.

  3. Help coordinate their move to include guiding them through the process of packing, downsizing, moving personal items, as well as the sale of the home if needed.  (Assisting in Veteran’s Benefits where applicable)

Home Patients 

PRIVATE FEE SERVICE

If the goal is to stay at home, we can help develop a long term plan to prepare for a crisis working together with physician recommendations in 3 easy steps:

  1. Meet the patient where ever they call home, assess concerns & needs.

  2. Educate on available resources based on individual need in conjunction with physician’s plan of care.

  3. Make referrals to area agencies & stay involved with patient/physician until resources are implemented in the home.  Follow up as needed.
